Document Checklist
You do not need everything on this list. Gather what applies to your situation before you call or visit a DFR office.
Proof of Identity
One of these
- Driver’s license or state ID
- U.S. passport
- Birth certificate
- Social Security card
- Military ID
Proof of Indiana Residency
One of these
- Recent utility bill (electric, gas, water)
- Lease or rental agreement
- Mortgage statement
- Mail addressed to you at your Indiana address
Proof of Income
All that apply
- Recent pay stubs (last 4 weeks)
- Employer letter stating wages and hours
- Self-employment records or tax return
- Unemployment benefit statements
- Social Security or SSI award letter
- Pension or retirement income statements
- Child support or alimony documentation
If you have no income, you may need to sign a statement saying so.
Household Information
For everyone in your home
- Names and dates of birth for all household members
- Social Security numbers (if available)
- Immigration documents, if applicable
Health Insurance Information
If you have or recently had coverage
- Current health insurance card and policy number
- Any employer-offered insurance details
Even if you have some insurance, you may still qualify for Medicaid.
Special Situations
Bring these if they apply
- Pregnant: doctor’s confirmation of pregnancy
- Disability: SSI or SSDI award letter, or disability documentation
- Foster youth: documentation of foster care history
